Losing a loved one is never easy. At Pantheon Life, we understand that this is a challenging time, and we’re committed to making the claims process as straightforward and compassionate as possible. Our dedicated claims team is here to guide you through every step and ensure your claim is handled with the care, professionalism and respect you deserve.

How to submit your claim

Claims can be submitted via email to claims@pantheonlife.co.za or by contacting us on 0860 101 038. We recommend submitting your claim as soon as possible to avoid any delays. Our team will respond promptly and keep you informed throughout the process.

Important information about your claim:

  • All claims (including all supporting documents) must be submitted in writing, within 12 months from the Insured Event. The claim will be forfeited and not honoured if the claim is not submitted successfully within this period.
  • Claim payments will be made into South African bank accounts only.
  • No claim will be considered (or Benefit/s paid out) if:
  • The Insured Event took place before the Acceptance Date.
  • The Claimant cannot provide acceptable documents as positive verification of the Insured Event and identity of the deceased.
  • If the Assured Lives do not fall within the definitions or parameters as indicated in this document.
  • The Insured Event took place within the applicable Waiting Period (Unless the Waiting Period was waived).
  • If a claim is fraudulent, or if any fraudulent means were used to obtain any Policy Benefit/s, the claim will not be honoured, and the Insurer has the right to cancel the Policy.
  • The Insurer has the right to investigate claims where risk indicators were triggered, which may affect the claim outcome and/or payment turnaround time.
  • Any outstanding Premium/s will be deducted from the claim amount.
  • If an Insured Event occurs outside South Africa, the claim will require official proof of death from the other country, which may not be verifiable. Therefore, payment of such claims cannot be guaranteed.

Required claim documents

You can have peace of mind knowing that all claims will be managed professionally, with care, compassion, and respect. Below, you’ll find the relevant claim documents by Benefit type.

For Primary Benefits:

  • Completed and signed claim form
  • Certified copy of the Claimant’s ID
  • Certified copy of the deceased’s ID
  • Death certificate
  • Notification of death: BI-1663/BI-1680 or DHA-1663/DHA-1680, completed by the doctor who certified the death
  • Police report (if death occurred due to accidental/unnatural causes)
  • 3 months’ stamped bank statement of the Claimant confirming the account holder and bank account
  • Any other documents, as required by the Insurer in its sole discretion.

For Capital Benefits:

  • Completed and signed claim form
  • Certified copy of the Claimant’s ID
  • Certified copy of the deceased’s ID
  • Death certificate
  • Medical Aid confirmation letter
  • Medical reports of the treating doctor, attending clinic and or hospital attended by the deceased

Death due to Unnatural causes, please include:

  • Statement of Police
  • Postmortem report
  • Death due to a motor vehicle accident, please also supply an Officer’s Accident report.
  • Notification of death: BI-1663/BI-1680 or DHA-1663/DHA-1680, completed by the doctor who certified the death
  • Stamped bank statement of the Claimant confirming the account holder and bank account
  • 3 months’ stamped bank statements of the deceased
  • Any other documents, as required by the Insurer in its sole discretion.
